Church Health Assessment
This is not a survey about attendance, budgets, or buildings. It measures whether your church is making disciples who make disciples — Jesus’ own scorecard. A large, well-funded church can score low here. A small, ordinary one can score high. That is the point. Score what is actually happening, not what you preach, hope, or intend. The gap between the two is exactly what this tool exists to show you.
Rate each area 1 to 5. Anchors are given at 1, 3, and 5 — use 2 or 4 for the spaces in between. Score the congregation, not yourself: where a question asks about “members,” answer for the ordinary people in your pews, not for you and your staff.
